We are in search of a.RN Clinical Care Coordinator* to supervise, provide and coordinate nursing care in compliance with facility and regulatory policies and procedures and to assess residents' care needs.

*Primary functions:*
* Interview residents, residents' representatives and others with information related to the Minimum Data Set (MDS) assessment.
* Collect all data necessary to complete MDS forms and related documentation.
* Make appropriate referrals to other health care professionals.
* Participate and/or conduct interdisciplinary meetings.
* Supervise and participate in the overall care of the residents
* Oversee clinical coordination, review labs, assist the physician with rounds
* Participate in the facilities Quality Assurance process to review facility systems as directed

*Wage Information:.
* $5,000 SIGN ON BONUS
* $37.15/hour -$41.65/hour based on experience ad an RN & LPN
* \*Including $0.85/hour MI pass through pay until 09.30.24
